Introduction

The Global Fund has been critical in the fight against HIV, TB, and malaria and has helped reshape global health since its inception in 2002. Over the past two decades, it has saved 70 million lives and reduced deaths from the three diseases by 63%.

Today, multiple interconnected and interlinked crises – including climate change, economic instability, threats of new pandemics, and conflicts – threaten the progress we have made against HIV, TB, and malaria. To end the epidemics and build a healthier, more equitable future, we must stand together to support proven partnership models. We must stand together to support the Global Fund. 

The Global Fund’s Eighth Replenishment Investment Case calls for US$18 billion to drive impact at scale. The Investment Case projects that a fully-funded Global Fund partnership would:

  • Save up to 23 million lives between 2027 and 2029
  • Avert 400 million infections or cases between 2027 and 2029
  • Deliver a return on investment of 1:19 with every dollar invested resulting in US$19 in health gains and economic returns
  • Save over $42 billion in primary health care costs between 2024 and 2029 t

The Global Fund invests one-third of its resources into strengthening health and community systems, including support to community health workers, disease surveillance, and supply chains. These investments improve health systems resilience to future pandemics and accelerate the progress towards universal health coverage (UHC). 

The Global Fund’s multi-stakeholder partnership model has been important in responding effectively to the three diseases, strengthening health and community systems, and promoting domestic financing for health in Africa.

The ‘One World, One Fight’ campaign is an urgent call to support the Global Fund. Civil society and communities across the region are calling for increased domestic resources for health and a fully-funded Global Fund to safeguard the gains made so far and save up to 23 million more lives.

  • A call to African leaders – invest in health and in the future of our continent: Policymakers and leaders must think beyond today’s challenges to the future of the world’s youngest continent and commit boldly – with the inspiring audacity of their youth – to ending the epidemics and building a healthy future for Africa and the world.
  • African governments are already stepping up: in 2024, 23 countries increased their health spending compared to 2023. Now is the moment for African leaders to continue the momentum with domestic ambition and action.

  • The Global Fund is the largest external funder for HIV, TB and malaria, but its resources cover only a fraction of the estimated US$140.6 billion needed globally between 2027–2029 to address the epidemics.
  • African governments must increase their health budgets to ensure sustainable financing for HIV, TB, malaria, and broader health systems. Financing health is an investment in long-term economic and social benefits. It is an investment, not a cost.
  • Countries have committed to implementing multiple regional and global frameworks on domestic resources for health, including the Abuja Declaration and the Lusaka Agenda. These pledges should be translated into domestic legislation, budget prioritization, and national-level accountability mechanisms through multistakeholder actions. 
  • The Global Fund’s co-financing policy supports countries to progressively increase investments in health  programmes. A minimum of 15% of Global Fund allocations are subject to co-financing requirements. The 2025 Results Report notes that co-financing commitments from countries increased by about 20% compared to the previous grant cycle. In addition, countries that receive Global Fund grant funding are increasingly becoming donors.

  • Every person has the right to quality health care and yet, 615 million people in the African region still lack access to the services they need. 
  • Investments in health are investments in justice and dignity for all. The Global Fund focuses on those most at risk, including women, girls, key populations, and marginalised groups, because even the best innovations won’t reach their potential for impact if people can’t access them. In 2024, 85% of pregnant women living with HIV in Global Fund-supported countries received treatment to prevent HIV transmission to their babies.
  • The Global Fund’s investments in community-led and community-driven programmes as well as its rights-based programming have improved access to HIV prevention, treatment for drug-resistant TB, and malaria interventions among vulnerable populations.

  • Community-led programmes are critical to reach all populations with services that are most relevant and responsive to their needs. They reduce barriers to services, strengthen systems of care, and improve health outcomes. 
  • The Global Fund is investing US$6.1 billion in health and community systems to accelerate progress against HIV, TB and malaria and boost global health security. The health workforce represents the largest share within this investment.
  • The Global Fund partners with communities in sub-Saharan Africa to design, implement and monitor programmes that respond to the needs and challenges of communities on the ground. In 2024, integrated community-led monitoring (CLM) initiatives were rolled out in Burkina Faso, the Democratic Republic of Congo, Kenya, Mozambique, Nigeria and the Philippines, covering HIV, TB, malaria, pandemic preparedness and climate resilience
  • The most successful health programmes leverage the strengths of communities as experts, and support their consistent engagement and leadership. The Global Fund supports long-term capacity building of community-led organizations to improve service delivery, address local needs and ensure equitable access to quality health care.
